Once you finish registration, we can only change your name or date of birth if you provide us with a government-issued ID.
Please provide us with a government-issued ID: Passport, Driver's license, or Divorce/Marriage certificate for our team to correct.
Please note, that if you have already completed the course, you may need to reach out to your state or provincial agency for their records to be updated as well.
Once we receive your document, we will be able to update your record. From there you will be able to order a replacement card from ILostMyCard.com.
